Reporting a bug
Because all processing happens in your browser, the browser is almost always the thing that explains the bug. Include the browser and version, the operating system, the source format of the file, and what you expected to happen. If a format button was greyed out, that is the browser declining to encode that format rather than a fault — mention it anyway, it helps map which builds support what. Please do not attach the image itself unless you are comfortable sharing it; its size and format are usually enough.
